  • Salary $85,665 – $99,241 per annum – Temporary Full Time (up to 30 June 2023) – AHP2

The Community Connections – Care Partner Program (CCCP) Program Coordinator applies clinical experience specialised clinical knowledge and professional competence to plan, implement and evaluate a comprehensive and integrated range of services, appropriate to the needs of the local community. The CCCP Coordinator works under the limited clinical supervision and may provide to less experienced Allied Health Professionals, Allied Health Assistants and Students. As a member of a multi-disciplinary team, including health professionals and service providers from other sector, the CCCP Coordinator utilises a combination of preventative early intervention treatment/therapy and evaluation approaches. The coordinator is responsible for effective and efficient implementation of the Community Connections – Care Partner Program (CCCP) in the Flinders Upper North Regional Health Service region in accordance with the Community Connections Program Guidelines. The coordinator ensures the recipients receive quality rehabilitation and restoration focused services which are appropriate to their needs and well-coordinated through effective case management. The coordinator also contributes to the quality and consistency of the Community Connections Care Partner Program, by ensuring all administrative reporting and evaluation requirements are met.

Must hold a recognised qualification within a relevant Allied Health profession (Social Work, Physiotherapy, Occupational Therapy) and be eligible for practicing membership of the relevant Professional Association. For those professions requiring Registration, all requirements to obtain and maintain current registration must be fulfilled. For self-regulated professions it is desirable to participate in the professional association accredited continuing professional development program.

Appointment will be subject to a satisfactory Criminal History Screening/Check and specific immunisation requirements.  COVID-19 vaccination is an essential requirement under the Public Health Act 2011 (Healthcare Setting Workers Vaccination) (COVID-19) to work in SA Health/health care setting.
