Job Description

We have an exciting opportunity for a Care Coordinator to join our team based in our Whyalla office.  The Care Coordinator will lead and coordinate the support services offered to NDIS eligible participants and disability clients in Whyalla. This will also include the coordination of case management and brokerage services.

Your responsibilities will include:

  • Providing professional direction and support to staff in the delivery of care and best practice to clients
  • Ensuring compliance and legislation is adhered to at all times
  • Prepare and distribute rosters with consultation with the MYP Systems Specialist and ensuring employees are allocated hours and paid correctly
  • Managing a team of disability support workers and ensuring theyare engaged, motivated and delivering quality care
  • Compile and prepare reports for management and other external stakeholders including but not limited to Government Departments

What you need to be considered for this position:

  • A minimum Certificate III in Individual Support is essential
  • Strong communication and interpersonal skills
  • Experience working with people with various barriers
  • Experience in the industry with a sound understanding of legislation and compliance
  • First aid will be highly desired
  • Ability to work unsupervised as well as part of a team
  • Able to deliver training to a team when required
  • Strong understanding and respect for Indigenous culture
  • Have a current open driver’s license
  • Hold a current or are able to obtain NDIS Worker Screen and DCSI (Child-related)
  • Hold a current or able to obtain a National Police Check
  • Australian Citizenship or Permanent Residency
  • Evidence of having received Covid Vaccination

Please note: The successful candidate will be required to participate in a National Police check, an NDIS Worker Screen and a DCSI Child related screening, prior to confirmation of appointment to the position. COVID vaccination is required for this role.
