ITC Care Coordinator – Full Time – Whyalla

Country & Outback Health’s extensive primary health footprint extends throughout regional and remote South Australia. We are seeking the right candidate to work in the Wellness Our Way Team, to provide practical support to Aboriginal and Torres Strait Islander people with chronic conditions to better manage their condition.

About the role:

The Care Coordinator contributes to improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through access to care coordination, multidisciplinary care, support for self- management and improving access to culturally appropriate mainstream primary care.

The position will:

The Care Coordinator will work in accordance with the client’s care plan, in consultation with the referring GP.

This includes providing feedback to the GP about how the client is managing their condition, the treatment of their condition, what services have been arranged for the client, and any other issues regarding the client’s health.

The Care Coordinator may also provide feedback to the GP about the client’s living environment when this information is relevant to the care plan, for example, noting home safety or access issues that have a health implication.

The Care Coordinator will assist clients to understand their chronic health condition and how to manage it, for example, this includes support for chronic disease self-management and assistance with care plan.

Qualifications:

  • CIV Aboriginal & Torres Strait Islander Primary Health Care
  • Be a passionate and dedicated team member
  • National Police Check
  • Department of Human Services Child-Related Screening
